What personal information do we collect on the Premins Site, and how do we collect it?

 

·         If you are a broker:

 

    • We will ask you for your name, address, phone number and your e-mail address.
    • If you want to make payments through the Premins Site, we also ask you for your checking account number, the name on that checking account, and the bank routing number.

 

·         If you are an insured:

 

    • We will ask you for your name, address, phone number, and e-mail .
    • If you sign up for automatic bill payment, we will ask you for the street address associated with your bank account, e-mail address, bank name, bank account number, bank routing number, and phone number.
    • If you sign up for payment by electronic check, we will ask you for the street address associated with your bank account, bank name, bank account number, bank routing number, and phone number. We will also ask you for your e-mail address, but it is not required.
    • If you wish to pay by credit card or debit card, before you enter any personal information, you will be redirected to the Web site of our credit-card processor, Official Payments, at www.officialpayments.com and treated in accordance with that site’s privacy policy. We do not collect on the Premins Site any personal information that you provide in connection with arranging to pay your premiums by credit card or debit card.

 

How do we use the personal information that we collect?

 

·         If you are a broker:

 

o   We will store your name, address, phone number and e-mail on the Premins Site.

 

o   We use your e-mail address to send you confirmations of payments and other transactions.

 

o   Your name, address, phone number, bank account number and bank routing number will be forwarded to our payment processor, Chase Paymentech (www.chasepaymentech.com), and treated according to that company’s online privacy policy. Chase Paymentech will send us back a confirmation number that we will use to identify you when you use the Premins Site. We will not store your bank information on the Premins Site.

 

o   The Premins Site automatically logs errors and various categories of diagnostic information.  All error log data is used only to resolve technical issues and will only be shared with technology partners that we may retain to help us maintain the Premins Site.  Diagnostic and error logs can contain any information entered into the Premins Site that resulted in a site error, so it is possible that your bank information may reside on the Premins Site momentarily if it shows up in an error log, but if that happens, it will be deleted.

 

·         If you are an insured:

 

o   We will store your name, address, phone number, and e-mail address on the Premins Site, along with other information from your account that is not personally identifiable information, such as the last four digits of your bank account number and your policy details.

 

o   If you sign up for automatic premium payment:

 

§  We will use your e-mail address to notify you before the scheduled date of any payment to remind you that your bank account will be debited in the amount of the payment due.

 

§  Your name, address, phone number, bank account number and routing number will be forwarded to Chase Paymentech, our payment processor (www.chasepaymentech.com) and treated according to that site’s privacy policy. Chase Paymentech will send us back a confirmation number that we will use to identify you when you use the Premins Site.

 

§  We will not store your bank information on the Premins Site.

Do any third parties collect personal information on the Premins Site?

No. We use Google® Analytics to track user activity.  Google tracks where you go on the Premins Site and combines that information with information that Google may have about you through your having a Google account and/or through your use of other Web sites that use Google Analytics.  Google and its affiliates may use cookies -- but *not* your name, address, email address, telephone number or any other information that identifies you personally -- to serve advertisements about goods and services of interest to you on the Premins Site and on others that you visit.
